Case Management
VRA provides Case Management service for clients with serious disabilities or catastrophic injuries. Our VRA case managers work in collaboration with the client, insurers, health-care providers and employers to facilitate a safe, respectful and expedient return to productive activity or work.
The case management process involves:
- Assisting the client to identify realistic and achievable objectives and a plan of action to achieve them.
- Assessing a client’s employment readiness and employment-related needs, such as retraining or graduated return to work plans.
- Advocating on the client’s behalf, and accompanying the client to meetings in academic and vocational settings to help to facilitate any necessary accommodations.
- Documenting the client’s progress with clearly written, narrative assessments and reports.
